We present two interesting and rare cases of pneumorrhachis (intraspinal air). The first patient is a 43- year-old male patient who had a road traffic accident and trauma CT showed impressive pneumocephalus and pneumorrhachis within the cervical canal. Treatment was conservative and the patient improved the following weeks with reabsorption of the air. Another 60-year-old female patient had known diverticulitis and developed paraparesis of her legs with associated fever. MRI of her spine showed extensive intraspinal air in the thoracolumbar space. She was operated combined with neurosurgeons and abdominal surgeons with colostomy and laminectomy. After a long treatment on ICU and rehab center 6 months after surgery she was able to walk with help.
