Abstract

A 70-year-old patient had basal cell carcinoma on right cheek diagnosed by biopsy and managed in 2009 by large resection of the tumor with marginal deep limit. An adjuvant radiotherapy was indicated; but the patient refused any additional treatment.

A 70-year-old patient had basal cell carcinoma on right cheek diagnosed by biopsy and managed in 2009 by large resection of the tumor with marginal deep limit. An adjuvant radiotherapy was indicated; but the patient refused any additional treatment.
Three years later, the patient returned with deep-seated and superficial disease in the same region with submandibular lymphadenopathy. After positive biopsy and negative lymphadenectomy, the patient still refusing radiotherapy and was lost to follow-up. Two years later, the patient returned with a huge ulcerated and bleeding mass of the right cheek infiltrating oral cavity, floor of the orbit and obstructing both nasal cavity (Figure 1). Sixty six Gy in 33 fractions were prescribed using RapidArc radiotherapy. The second image shows the immediate complete response to irradiation therapy (Figure 2). Unfortunately, the lesion recurred a year later and actually the patient is candidate to reirradiation protocol.
