Abstract:

Introduction: The upper third of the face has a signifi cant effect on inducing a sense of beauty, youth and health to the viewer and therefore in recent decades this area has been the target of many different rejuvenation considerations (surgical and non-surgical). Due to the close connection of the forehead complex - Eyebrow-eyelid, achieving optimal results will require simultaneous treatment of all three elements. Although today the endoscopic lift method has achieved a favorable position in forehead rejuvenation, but the need for advanced and expensive equipment long learning curve and technical limitations In men with long foreheads, or forehead or men with baldness, the male pattern is one of its weaknesses. In the present study, a minimally invasive technique with blepharoplasty-temporal approach was investigated as an effective alternative. Patients and study methods: In the period of 2019-2020, 31 patients were admitted to the plastic surgery clinic of 15 Khordad Medical Center based on Exclusion and Inclusion criteria. After obtaining informed consent and ethical considerations of the research, a standard questionnaire based on history and physical examinations Completed. Photograph and preoperative preparations were performed and patients underwent surgery. The surgical approach was performed by blepharoplasty and temporal incision with dissection in the subperiosteal plane. The results of the intervention were evaluated at least one, three and six months later. The action was based on objective and subjective criteria. Results: In most cases, the surgery was performed under general anesthesia, but as day care and without the need for hospitalization. The average duration of surgery was 135 minutes and the average follow-up time was 14 months. The main anthropometric criteria including medial canthus-eyebrow distance MCBD increased by 2.2 mm and lateral canthus-eyebrow distance LCBD increased by 1.5 mm. Good to excellent vertica frown linesimproved good to medium forehead lines and medium crow's feet lines were evaluated. The result was an overall beauty in the upper third of the face and patients' satisfaction with the operation Conclusion: Due to the close relationship between the components ofthe upper facial complex, rejuvenation with the trans-blepharoplasty and temporal surgical approach is a very effective, safe and minimally invasive method that can simultaneously treat and correct problems in the forehead, eyebrows and eyelids. This technique is strongly recommended in people with long or prominent foreheads or a masculine pattern baldness without signifi cant complications or scars
