The data of 58 patients affected by congenital blepheroptosis with poor to good levator function that underwent unilateral or bilateral levator advancement through the standard approach were included. Final outcome measures included postoperative eyelid height, contour, and symmetry. Results: The degree of ptosis was mild to moderate in 39 patients (67%), while it was severe in 19 patients(33%). Maximum levator resection was performed in all patients with severe ptosis and in patients with moderate ptosis with poor levator function. The patient satisfaction was 85% for those with fair to good levator function and 50% for those with poor levator function.
