Optimization of rhinoplasty by curved dorsal septal de-humping and restoring natural anatomical structures

Abstract:

As rhinoplasty is one of the most requested cosmetic interventions in our private practice and even in the region and the world. Optimizing the results and giving the patients what they want is of prime importance to guarantee minimal complains after surgery and to minimize revisions. In this article we discussed what characterize the noses we are operating on and to back to the well know tripod theory and what we call (tent pole theory) to prevent one of the most dreaded consequence after rhinoplasty (tip ptosis) in thick skinned subjects and to stress on the importance of restoring the natural anatomical relations of the lower lateral cartilage and the upper lateral cartilage in shaping the nose in a better way that satisfies our patients. The review was conducted by comparing the postoperative results in the period from 2016 till Jul’ 2018 that included the patients operated by the authors. The authors used to do open approach rhinoplasty under general anaesthesia. Two hundred rhinoplasty cases was included in this review, male to female ratio was (7:33), the surgery was conducted in two private hospitals in Baghdad, under general anaesthesia and a standard hypotensive technique, average operation time was 72 min, cast and stiches were removed after seven days. The patients were followed up in regular intervals of bi-monthly after the first month after surgery. Although it might take a little longer operation time (average of 72 min) and a tedious work to reframe every tissue in place, yet it gives a very nice results with minimal complains and maximizes patients’ satisfaction and boost up surgeon’s confidence and certitude, and reproducibility of utmost results.

Biography :

Mohamed R Alanbari has completed his Post Graduate study and received the Iraqi Board Degree from the Iraqi Board of Medical specialization in Baghdad, and he quit public sector and run his own private plastic surgery clinic in Baghdad, Iraq. He is an Associate Member of ISAPS, and a Member of ASPS. The authors work as a team in a private plastic surgery clinic in Baghdad, Iraq.
