Role of fat grafting in Breast Deformity Corrections

Abstract:

A lot of deformities after breast augmentations appeared. Tebetts in 2009 and Leroy inm 2006 stablish that after breast augmentation 23.5% of breast augmentations will be re-operated. Then from this patients, 35% will require another surgery, but the most astonishing is that from these patients, 32% will be reoperated in less than 5 years.

Material and Methods: According with these data, we see in consultations a lot of secondary breast deformities, from mild to severe. The age were from 35 to 55 years old. Previous markings developed by us, with the patient in sat position with hands up and down, We used general anhestesia.The implants were changed, developed a new plane to insert the new implants, either under subglandular or sub muscular. Sutures with vicryl 2-0 from subglandular, to the muscle were placed in order to displace central or superiorly the nipple areola complex. The inferior abdomen were prepared and infiltrated with saline solution 100 and epinephrine 0,1 ml in order to decrease bleeding. We not use xylocaine because has been reported that diminishes the stem cells viability. We took fat from the hypogastrio with 10 ml syringes and cannula 2.5 ml with 6 holes in order to diminishes trauma to the fat. Then were decanted and the blood and saline solution were discarded. With the same cannula size, were places in subcutaneous plane in the inferior pole of the breast, average 40 to 80 ml on each side. The Drainage were placed in more that 80% of the patients. The new sub mammary fold were fixed with 2-0 vycril. At the end of the surgery, the patient was sat to see symmetry of the breasts. Special brassiere were placed that was used for 1 month day and night. Patients were discharged same day post-surgery.

Biography :

Dr. Carlos Oaxaca has completed his PhD at the age of 24 years old from Universidad Nacional Autonoma de Mexico and his specialty in Plastic and Reconstructive Surgery from same University. He is member of American Society of Plastic Surgeons (ASPS), International Federation for Adipose Therapeutics and Science (IFATS), European Society of Rhinoplasty, Mexican Association for Plastic and Reconstructive Surgery where he is General Coordinator for Scientific Committee, Member for Mexican Council for Plastic and Reconstructive Surgery, President of Interactive Plastic Surgery Network and works in Private Practice for 28 years. He has published several articles as so-author an chapter of Aesthetic Book in Europe.
