The splenectomy in special cases

Abstract:

The purpose of this presentation is to highlight the feasibility of laparoscopy in spleen trauma and to present the case of a patient with extreme splenomegaly, where the spleen occupied the entire abdomen. The first case is that of a boy 11 years old fallen from a height of 6 meters, with a rupture of spleen in the hilum. Although laboratory tests were normal at admission, immediate surgical decision making allowed to finish the operation via laparoscopy, performing the four trocar technique (video presentation). Inpatient stay was five days, the child being discharged after vaccination, to prevent postoperative severe infections. The second case is that of a 63 years old patient with hairy cell leukemia, whose spleen stretched from the diaphragm to the cervix. Although during preoperative treatment it was not possible to obtain an acceptable coagulation correction, the operation went without inoperative incidents. These two cases emphasize the importance of adapting the specific surgical technique to each individual patient, judging every single case in detail, that of a good collaboration with the haematology department as well as that of surgical decision making to get the best possible results.

Biography :

Mihaela Le�?�?e has completed her PhD from Iuliu Ha�?£ieganu University of Medicine and Pharmacy Cluj-Napoca, Romania and Postdoctoral Studies from University of Medicine and Pharmacy, Carol Davila Bucharest, Romania where she obtained her degree in Advanced Laparoscopic Surgery and Surgical Oncology. She is the Chief of the operating block at the Baia Mare Emergency County Hospital, Romania. She has published the monograph “Abdominal drainage” and published more than 20 papers in reputed journals. She has been serving as an Editorial Board Member of repute. She is also Co-author of two important medical records: The Resident's Book of Surgery and Palliative Care at Home.
